Is it disgusting for men bunking with another guy to piss with the bathroom door open? when no nudity of any kind is visible, but toilet can be seen from bed but not when pissing is occurring

I'd place money on this happening during Kuno and Gr's adventures through time and space.
Anyway, if no nudity is visible, I don't see a problem. So I'd say no.

Closing the door isn't really a big deal if nothing's visible, beyond making sure someone doesn't accidentally walk in/the door doesn't drift open, but it's obviously going to bother some people and it doesn't benefit the person leaving it open in any way so there's really no reason not to be closing it. I'd side with the person insisting it's closed, even if it doesn't bother me specifically, because the person not closing it is being needlessly annoying.

Entirely depends on the level of intimacy/familiarity between the two. Father and son? No problem. Friends? Gray zone but I'd probably go with "not a problem" if they've known each other for years. Strangers who are only co-bunking because they've been assigned to (e.g. boys at a summer camp sharing the same cabin)? Yeah, kinda weird / inappropriate.

I don't know why you're splitting hairs over immediate family ... and I also don't know why you would say that if you were to split such hairs that you would place brothers before fathers. To me it would be more awkward (barely, but if I had to pick it'd be the more awkward of the two) for my brother to see me taking a piss than for my father to see me. The one raised me from birth, cleaned me when I was a baby, bathed me, etc etc. The other did not.
Also, just throwing this out there: we never heard what angle the toilet lies in to the door or how far it juts out from the wall so that the user of the toilet can be seen through the doorway. That could change some of these votes. There's a difference between a toilet that can clearly be seen through the door and is at a 90° angle (so you can see the guy from the side, including him holding his private) and one which is at a 0° angle to the door (so you could only see his backside) or which is recessed (so that even if you're seeing his side profile you're only seeing the posterior half of it).

It's never ok to leave the door open unless you're by yourself in the hotel room/house/theme park/etc.

I think it's fine if nothing is visible. It's similar to using a urinal. Those don't have doors.
